Check in the app's Settings → Notifications that permission is granted (you can open iOS Settings from there). Also check that no Focus mode silences Dose Minder; reminders are Time Sensitive, so you can allow them in each Focus. Open the app now and then: if you have not opened it for many days, you will get a reminder to do so, so it can keep scheduling notifications.

Both people need an iPhone with iCloud turned on. Go to Settings → Patients, tap the patient and then Share…. Choose whether to share the whole patient or only some medicines, and the permission: Full access or Only mark doses. Tap Invite and send the invitation through Messages, Mail or another app. The other person just opens the link.

Changes travel through iCloud, usually within seconds. iOS may delay them if the other iPhone is offline, in Low Power Mode or low on battery. Opening the app syncs it and shows who marked the dose and when.

Each person keeps a full copy of what they had (medicines, schedules and history) as their own patient. From then on the copies no longer sync with each other.

Check that you are online and signed in to iCloud, and that your iCloud storage is not full. For a shared patient, what is saved counts against the iCloud of the person who shares it: if their iCloud is full, the app tells you. Your changes are not lost; they are sent as soon as possible.

Try good lighting with the whole prescription in the picture, or the original PDF. The app never makes data up: if it does not recognize something, it leaves the field empty for you to fill in. You can always add the medication by hand.

In the app, Settings → Delete all data. The privacy policy also explains how to delete the iCloud copy.
